Insurance Plans
Our preferred insurance company partners are Humana, Medicare and United Healthcare. We also accept Arizona Foundation, Blue Cross, CIGNA and most major plans. We accept many Medicare Advantage plans.

Dr. Whitely received her medical degree from The Ohio State University School of Medicine in Columbus, Ohio and completed residency at St. Joseph’s Hospital and Medical Center in Phoenix. Phoenix Magazine has listed Dr. Whitely in their annual “Top Doc” issue numerous times. Dr. Whitely founded Healthy Outlook Family Medicine immediately after her residency. A passionate educator, Dr. Whitely not only teaches area residents, she also frequently presents health care topics to senior programs, Phoenix College, Mended Hearts, and other area groups. Dr. Whitely was the President of the Arizona Medical Association from 2019-2020 and continues to serve as a Board Member. She loves reading, traveling, and scuba diving as well.
Dr. Cohen earned her medical degree at the Case Western Reserve School of Medicine in Cleveland, Ohio and completed her specialty training in Family Medicine at Banner Good Samaritan Medical Center in Phoenix. Dr. Cohen has been a Phoenix Magazine annual “Top Doc” for many years! Dr. Cohen enjoys helping patients with cosmetic medicine in addition to practicing full-spectrum family medicine. Dr. Cohen joined Healthy Outlook Family Medicine in 2003. Dr. Cohen is an avid runner and has completed several full marathons. She also enjoys traveling, reading, and playing piano.
Melissa got a BS in Physiology at the University of Arizona. She received her Masters in Physician Assistant Studies from the Midwestern University College of Health Sciences. Before coming to Healthy Outlook, she worked as a Physician Assistant in the far West valley. Melissa sees same day sick patients and assists with Comprehensive Visits and Annual Wellness Visits.
Sara grew up in Illinois. She received her nursing degree in Illinois in 2010 and worked in the Medical-Surgical unit. She moved to Arizona in 2018. She received her Nurse Practitioner Degree from Chamberlain University. She likes to garden, hike and spend time with her kids and family.
Jennifer grew up in Michigan. She completed her nursing degree in Michigan in 2001 and did her masters in nursing at Walden University in Minnesota. She has lived in South Dakota and Colorado and moved to Arizona in 2021. She enjoys snowboarding, biking, and hiking.
